
Dr. Scott Gustafson, DVM, DAVCS
Board-certified Surgeon
Dr. Gustafson graduated with his DVM from Oregon State University and Washington State University in 1985 and completed his surgical residency and obtained an MS in veterinary surgery from Colorado State University in 1989. He became a Diplomat of the American College of Veterinary Surgeons in 1990. Dr. Gustafson taught small animal surgery at Oregon State University and was a key figure in developing the small animal surgery curriculum. In addition to owning the Surgery 4 Pets mobile practice, he also is a partner of Olympia Veterinary Specialists, a specialty hospital in Olympia, Washington, where he also performs surgery. Dr. Gustafson specializes in soft tissue, orthopedic, arthroscopy, thoracoscopy, laparoscopy, and cancer surgeries, with special interest in minimally invasive surgery and therapeutics. He has a young Border Collie named Syr and spends his free time enjoying the outdoors and sailing.

Dr. Nancy Leveque, DVM, DAVCS
Board-certified Surgeon
Dr. Nancy Korenek-Leveque received her veterinary degree in 1985 from Louisiana State University. Upon graduation she worked in private practice in Louisiana for two years before entering a three-year veterinary surgical residency at the University of Tennessee in 1988. Following completion of her residency in 1991 she became board-certified by the American College of Veterinary Surgeons. Dr. Leveque has worked as the staff surgeon for private small and large animal practices in Texas and Washington before settling in Oregon in 2016. Dr. Leveque’s expertise encompasses a wide range of orthopedic and soft-tissue surgeries. She currently lives in Tualatin with her husband and 17yr old daughter with their cat and three dogs. She spends her free time with her daughter and husband in numerous family activities and enjoys playing tennis, traveling, gardening and horses.

Dr. Pam Fulkerson, DVM, DACVA
Board-certified Anesthesiologist
Dr. Pam Fulkerson graduated from Colorado State University with her DVM in 1988. She completed a residency at Washington State University in 1990-1992, working in a myriad of small animal practices and performing research prior to becoming a Diplomate of the American College of Veterinary Anesthesia and Analgesia in 2006. Her special interests include pain management with a dedication to improving the general quality of veterinary anesthesia. Dr. Fulkerson runs high risk anesthesia cases for the mobile practice, as well as high risk cases for a range of veterinary hospitals in the Willamette Valley and Olympia Veterinary Specialists. Dr. Fulkerson also co-owns Surgery 4 Pets and is a partner with Dr. Gustafson in both the Surgery 4 Pets mobile practice as well as Olympia Veterinary Specialists. In her free time, she loves to hike with her dogs and enjoys playing the piano and dancing.

Michael, CVT
Certified Veterinary Technician
Michael has been in the veterinary field for over 30 years. He had a humble beginning as a kennel assistant at a small animal hospital in Southern California where he fell in love with the profession and working with animals. He worked as a veterinary assistant in New Mexico and in 2003, he moved to Oregon to work at a surgical practice in Portland. In 2004, he earned his credentials and graduated tech school with his CVT license. He has been with the Surgery 4 Pets mobile practice for 5 years and has worked closely with Dr. Gustafson for 11 years. He lives in Portland and enjoys exercising and spending time with his feline companion named Abby.

Tori
Veterinary Assistant
Tori recently moved to the PNW from California's Central Coast to pursue vet school. She grew up with a love for animals and cared for a wide array of pets, including cats, dogs, pocket pets, reptiles, birds, and horses. She has been working with the S4P team since October 2019 and her experience comes from working as a surgical assistant and scrub tech for several years at a specialty hospital in Santa Cruz, California. She lives in Corvallis with her boyfriend and enjoys hiking with their dog Blue and riding horses in her free time.
